Initial RN Assessment
Our Initial RN Assessment Services ensure safe, compassionate care transitions for children, seniors, and post-hospital patients. All visits include a licensed RN providing a full assessment, documentation, and personalized care support.
Pediatric RN Visit (Virtual or In-Home) –

Duration: Up to 2 hours
Includes:
- Pediatric vitals and symptom assessment
- Family education and care recommendations
- Virtual or in-home flexibility
- $150/hour after initial 2 hours
Elder Care RN Visit (Virtual or In-Home) –

Duration: Up to 2 hours
Includes:
- Full vital signs review
- Medication reconciliation
- Cognitive/behavioral wellness check
- $150/hour after initial 2 hours
Hospital-to-Home RN Transition Visit –

Duration: 2 hours | Coverage: 30-mile radius from hospital
Includes:
- Hospital pick-up and transport to home
- Discharge instruction review
- Home setup guidance and 2-hour RN assessment
- Coordination with caregivers or case managers

- Are supplies included? **Yes. ** All essential RN supplies are included in the visit fee.
- Is transportation private RN vehicle or medical van? **Medical van. ** All hospital pickups are conducted using ADA-compliant medical vans.
- Is family education included? **Yes. ** We provide personalized family education and care instructions during each visit.

Founded by Sabrina Putney, RN, BSN, MSN, Ret. US Navy, RN Harbor Care brings clinical expertise, military precision, and heartfelt service to families across the DMV (DC, Maryland, Virginia) region.

Our licensed RNs provide bedside-level care inside ADA-compliant mobile vans and in-home visits, backed by decades of experience and compassion.
